Hipotiroidismo por tiroidite autoimune ou de Hashimoto.
A doença Hipotiroidismo por tiroidite autoimune ou de Hashimoto autoimune caracteriza-se histologicamente pela presença de infiltrado linfocítico no tecido tiroideu (tiroidite linfocítica crónica).
Clinicamente pode cursar com tiroide atrófica, normal ou bócio e analiticamente manifesta-se pela presença de anticorpos antitiroideus (anticorpos antiperoxidase e anticorpos antitiroglobulina, ou anti-TPO e anti-Tg, respetivamente).
Em casos raros pode haver ausência de anticorpos por insensibilidade do ensaio laboratorial ou por processo autoimune totalmente intratiroideu.
A autoimunidade é responsável por mais de 90% dos casos de hipotiroidismo não-iatrogénico em zonas sem défice de iodo. Uma variedade de fatores genéticos contribui para o aparecimento de tiroidite autoimune.
Contudo, estudos epidemiológicos recentes sugerem uma forte influência de fatores ambientais no aumento da sua prevalência. A prevalência de tiroidite autoimune aumenta com a idade, sendo superior nas mulheres.
O risco de progressão de hipotiroidismo subclínico para hipotiroidismo clínico está relacionado com o valor de TSH e a presença de anticorpos antitiroideus (4,3%/ano em doentes com anticorpos anti-TPO positivos vs 2,6%/ano sem anticorpos anti-TPO elevados).
Perante um hipotiroidismo primário de causa não esclarecida, está indicado o doseamento de anticorpos anti- -TPO e anti-Tg, uma vez que a tiroidite autoimune é de longe a causa mais frequente. Uma vez efetuado o diagnóstico, a sua repetição não é necessária.
